The goal of our personal home care services is to enable elderly and/or disabled clients to live in the comfort of their own homes and achieve the highest level of independence possible. Our clients have the choice and flexibility as to how their care is delivered. Our non-medical home care service improves the quality of life for clients. We are an approved Medicaid provider in Arizona and New Mexico and we also provide private pay services for those that are not eligibility for Medicaid.

Our attendants must pass a background check and meet all state & federal requirements to become a personal home care services attendant, including current certifications in CPR & First Aid.

Steps to Receiving Quality Home Care

ARIZONA

Referrals – Referrals are made by CHR’s, discharge planners, physicians, Community Health Nurses and other concerned parties.

ALTC Application – The Arizona Long Term Care Services (ALTCS) program is for ages 65 and over that are blind or disabled and who need ongoing services at a nursing facility level of care. However, program participants do not have to reside in a nursing home.

Eligibility – The income limit for this program is $2,000 per month. Persons with Arizona Health Care Cost Containment System (AHCCCS) approved income may have income in excess of $2,313 if held in trust.

Home Assessment – A State ALTCS Representative will make a home visit to determine if you are eligible for ALTCS by meeting health and financial requirements.

Tribal Long Term Care – When determined eligible, a Tribal ALTCS Case Manager will be assigned to the client and will make a home visit to determine needs requiring assistance in activities of daily living. The case manager will create a service plan that lists what services the client will need and how many hours per week an Attendant will work for the client.

Home Care Services – The case manager will allow clients to choose a home care agency to provide an attendant to fulfill the duties determined necessary by the case manager. If a client chooses quality home care, a client service coordinator will visit the client a minimum of once a quarter to ensure that all services are being received and that the client’s needs are being met.

Final Step – The client’s referral is submitted to QHC billing department. Client intake and attendant hire package is completed. Once services begin, timesheets are submitted weekly to the QHC office every Saturday by 5:00 p.m. Client services coordinators, compile all timesheets and complete the TSCOL every Monday. The billing department bills each timesheet and payroll enters hours for the attendants to be paid for their services to the clients.

Verification of Eligibility – Medicaid eligibility will be verified by QHC to ensure that the consumer is eligible to receive personal care services. The consumer or their representative can come to the QHC Office or call to request for services at 505-726-0200 or Toll Free 1-800-726-1822. Income requirement: $1,387 per month.

Home Assessment – The MCO’s Care Coordinator will make a home visit to complete a home assessment form to determine the needs of the consumer regarding their “Activities of Daily Living” and complete a personal service care plan for the consumer.

Utilization Review – A completed personal service care plan and a home assessment form is sent to utilization management (UM) approved by the State of New Mexico Medicaid program and Centennial Care. The utilization management will determine if the medical necessity and the Personal Care Service Plan meet the criteria for service.

Home Visit – QHC Client Services Coordinator will assist with developing Consumer’s Individual Plan of Care (IPOC) according to the approved “Prior Authorization” and complete necessary paperwork to start services.

Personal Home Care – Upon completion of the consumer intake form and the attendant’s hire paperwork, an attendant is hired to fulfill the duties set forth in the personal home care service plan. Client services coordinator will visit the consumer a minimum of once a month to ensure that all services are being received and that the consumer’s needs are being met.
